Dear Rabbi, as it is clear from your explanation, i wanted to make sure.
Is it your opinion that bread made from grounded wheat sprouts alone with no flour, that the Bracha recited is hamotzi-bircat hamazon and not shehakol- bore nefashot?

Yes you understood correctly; it is simple logic that bread which is made out of the five grains is considered bread upon which the blessing is Hamotzie & Birkat Hamazon. The answer is based on the quoted detailed sources which were also discussed with great Talmidei Chachamim. I am not aware of any source that mandates that the grain must turn into flour during the process of making bread.
